Dear Parents,

This is a reminder that McCormick Hospital will be coming to CMIS on Friday 31 October from 2:30pm to administer flu vaccines. Please complete the attached consent form:

for each person who will be having the flu vaccine and drop at the Health Office. If you have not yet booked your vaccine and would like one please complete the form and let us know as soon as possible. The cost of the vaccine is 450B per person. You will need to pay the fee directly to McCormick Hospital staff on the day.
